The American College of Cardiology (ACC) is pleased to announce a partnership with Philips Healthcare on a new Hospital to Home (H2H) Learning Destination to be featured as part of the ACC 60th Annual Scientific Session & Expo, held April 3 – 5, 2011, in New Orleans.

"Gaps in communication between cardiovascular professionals and the lack of interoperability between health IT systems make it very difficult for providers to deliver high-quality, patient-centered, coordinated care," said ACC President Ralph Brindis, M.D., M.P.H., F.A.C.C. "Our partnership with Philips to create this interactive, educational exhibit will demonstrate best practices for reducing hospital readmissions, as well as improving the quality of life for cardiac patients and their caregivers through engagement of providers, patients and caregivers at each point along the care continuum."

In the Hospital to Home Learning Destination, you can walk through simulated hospital and home environments featuring hands-on demonstrations showcasing best practices and the latest clinical and technical advances to reduce hospital readmissions. Learn about what can be done in the hospital before a patient is discharged to reduce the risk of readmission. You will hear from industry leaders, offering innovative products and technologies for the home and get a glimpse into the future of connected health solutions.

The adjoining Hospital to Home Theater will feature clinically based presentations, as well as discussions with physicians and healthcare providers currently utilizing hospital and home-based services. Speakers will address topics ranging from improving gaps in communication between cardiovascular professionals, to strategies to improve medication adherence, to the importance of interoperability between health IT systems. Hear from those on the front lines and learn how they are implementing new technologies and processes to increase patient compliance and reduce readmissions.

"The H2H Learning Destination complements what the College has been doing over the last several years to help cardiovascular professionals put evidence-based guidelines and Appropriate Use Criteria to work in their practice," said Harlan Krumholz, M.D., F.A.C.C. "ACC.11 attendees will be able to witness first-hand some of the options for reducing costly, cardiovascular-related hospital readmissions — an issue the College has made a priority through its own H2H initiative."
